Care Coach I (Hybrid)
Charlotte & Lee Counties, FL
Pay Rate: Up to $26.75
Schedule: Monday – Friday | Full-Time
Work Environment: Hybrid (Home-Based with Local Travel)
About the Role
We are seeking a compassionate and motivated Care Coach I to support Medicaid members throughout Charlotte and Lee Counties, Florida. In this hybrid role, you'll combine remote work with frequent in-person visits to members in their homes, assisted living facilities, long-term care facilities, and other community settings.
As a Care Coach, you'll serve as an advocate and resource for members by assessing their needs, developing personalized care plans, coordinating services, and connecting them with community resources to help improve their overall health and quality of life.
Responsibilities
- Conduct in-person visits with Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings (75–90% local travel)
- Complete comprehensive assessments to identify medical, behavioral, and social needs
- Develop and manage individualized care plans based on each member's goals
- Coordinate healthcare services with providers, community organizations, caregivers, and interdisciplinary teams
- Monitor member progress and adjust care plans as needed
- Identify and address barriers to care, including access to healthcare, transportation, housing, and community resources
- Educate members and families on available services and support programs
- Maintain accurate and timely electronic documentation of all member interactions
- Promote quality outcomes by ensuring members receive services in the least restrictive and most appropriate care setting
Required Qualifications
- Must reside in Charlotte County or Lee County, Florida
- Bilingual in English and Spanish (required)
- Bachelor's degree in a Health & Human Services-related field
- Minimum of 2 years of experience in healthcare, social services, care coordination, case management, or a related field
- Strong verbal, written, and interpersonal communication skills
- Proficiency with Microsoft Word and Excel
- Comfortable working with multiple electronic systems and documentation platforms
- Valid driver's license and reliable personal vehicle
- Proof of personal auto liability insurance meeting minimum coverage requirements (100/300/100)
- Ability to travel locally throughout assigned counties (75–90%)
- Ability to successfully complete a Tuberculosis (TB) screening
- Reliable high-speed internet for a home office (minimum 10 Mbps download / 1 Mbps upload; satellite and wireless internet are not permitted)
Preferred Qualifications
- Experience working with Medicaid and/or Medicare populations
- Previous case management or care coordination experience
- Experience documenting in electronic health record or case management systems
- Background in health coaching, wellness, or population health
- Knowledge of local community resources, social service agencies, and healthcare programs
