Care Coach I (Hybrid)
Highlands County, FL (Sebring & Avon Park Area)
Pay:
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Employment Type: Contract
Travel: 75–90% Local Travel
What You'll Do
As a Care Coach I, you'll serve as an advocate and resource for members by helping them access the care and services they need to live healthier lives.
Care Coach Responsibilities include:
- Conduct comprehensive assessments to identify members' healthcare and social needs
- Develop and manage individualized care plans
- Visit members in their homes, assisted living facilities, long-term care facilities, and other care settings
- Coordinate care with healthcare providers, caregivers, and community organizations
- Identify and help resolve barriers to healthcare and wellness
- Connect members and families with community resources and support services
- Monitor member progress and update care plans as needed
- Document all member interactions accurately in electronic systems
- Promote positive health outcomes through education, communication, and ongoing care coordination
Care Coach Qualifications
Required
- Bachelor's degree
- Minimum of 2 years of experience in healthcare, social services, case management, care coordination, or a related field
- Strong communication and interpersonal skills
- Proficiency with Microsoft Word and Excel
- Experience using electronic documentation systems and healthcare software
- Valid driver's license
- Reliable personal vehicle with minimum liability insurance of 100/300/100
- Ability to travel locally throughout the Highlands County area (75–90% of the workday)
- Ability to complete a Tuberculosis (TB) screening
- Passion for improving member experiences and healthcare outcomes
Preferred
- Master's degree
- Experience working with Medicare and/or Medicaid populations
- Previous case management or care coordination experience
- Experience documenting in electronic health record or case management systems
- Health coaching, wellness, or population health experience
- Knowledge of community resources, healthcare organizations, and social service agencies
Travel & Schedule
- Hybrid position with work performed remotely and in the field
- Monday–Friday, 8:00 AM – 5:00 PM
- Travel throughout the Highlands County area, including Sebring and Avon Park
- Frequent visits to member homes, assisted living facilities, and long-term care facilities
- Eligible mileage reimbursement for approved travel
