Care Coach

Pay Rate:
The Care Coach 1 is responsible for assessing and evaluating member needs to help achieve and maintain optimal health and wellness outcomes. This individual works directly with members and their families to identify barriers to care, coordinate resources, and develop effective care plans.
Care Coach Responsibilities:
· Conducting member assessments
· Developing and managing individualized care plans
· Coordinating community and healthcare resources
· Monitoring member progress and outcomes
· Identifying and resolving barriers to care
· Documenting member interactions and care activities
· Collaborating with healthcare providers and support services
Care Coach Qualifications:
· Bachelor’s Degree
· Minimum 2 years of related healthcare, social services, case management, or care coordination experience
· Strong Microsoft Word and Excel skills
· Experience using electronic systems and software applications
· Ability to manage multiple priorities independently
· Previous case management or care coordination experience
· Experience documenting in multiple electronic health record or case management systems
· Health coaching, wellness, or population health experience
· Knowledge of community health organizations, social services, and available local resources
