What You’ll Do:
The Care Coach plays a vital role in assessing and supporting members’ health needs to promote optimal wellness. This position facilitates member access to appropriate care resources, resolves barriers to effective treatment, and ensures ongoing care planning through regular assessment and communication.
Key Responsibilities:
- Assess and evaluate members’ needs, guiding them and their families toward appropriate healthcare and community resources.
- Apply a variety of care management strategies and techniques to support members with complex health issues.
- Monitor patient progress toward care goals through regular assessments, data analysis, and communication.
- Identify and resolve social, behavioral, or logistical barriers to care.
- Collaborate with internal teams and external providers to develop and maintain active care plans.
- Utilize electronic documentation tools to maintain accurate, timely, and compliant case records.
- Make informed decisions within established procedures, regulations, and professional standards.
Required Qualifications:
- Bachelor’s degree in a health-related field.
- 2+ years of experience in care coordination, case management, or a related healthcare role.
- Proficiency in Microsoft Word and Excel.
- Strong verbal and written communication skills.
- Comfortable using various electronic information systems and software platforms.
- Valid driver's license and personal vehicle liability insurance (minimum limits: $100,000/$300,000/$100,000).
- Must pass Tuberculosis (TB) screening.
- Demonstrated passion for improving patient and consumer health outcomes.
Preferred Qualifications:
- Master’s degree (e.g., Social Work, Public Health, Nursing).
- Prior experience with Medicare and Medicaid populations.
- Experience documenting in multiple electronic case management systems.
- Background in health promotion, coaching, and wellness initiatives.
- Familiarity with community-based health organizations and social service agencies.
